Jill Collins

Director, Pennyrile Area Agency on Aging & Independent Living / Aging & Disability Resource Center
Jill is responsible for the overall planning, development and implementation of all aging and disability programs and other related services for the Area Agency on Aging and Independent Living. This includes developing the Area Plan, Request for Proposal for service procurement, development of program budgets and contract oversight. Jill also completes administrative monitoring with subcontractors and provides training and technical assistance to staff. She is available to speak to civic groups and other agencies regarding programs and services upon request.

Amy Baumhofer

Social Services Case Manager
Amy provides case management to clients enrolled in the Participant Directed Services Program/Home and Community Based Waiver.  She meets with clients in their homes to conduct program visits and training.  She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Michele Braun

Program Support Specialist
Michele provides administrative and programmatic support for the multiple programs within the Area Agency on Aging and Independent Living. She conducts intake, assessments and outreach within the nine counties of the district.

Sylvia Chase

Social Services Case Manager
Sylvia provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high quality services are provided in a timely and cost effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs.  Sylvia covers Hopkins County.

Angela Gore

Administrative Assistant
Angela provides administrative support to the Aging and Independent Living staff members. She maintains the mailing directory for numerous councils.  She also attends and takes minutes for all program committee meetings. Angela prepares correspondence and is responsible for the daily maintenance of the Home Care and Title III Client charts.

Aleasha Fowler

Social Services Case Manager
Aleasha provides case management to clients enrolled in the Participant Directed Services Program/Home and Community Based Waiver. She meets with clients in their homes to conduct program visits and training. She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Ali Jones

Aging & Disability Resource Center Coordinator
Ali is the primary contact for all referrals, intake and eligibility pre screenings for clients accessing services. She also maintains a regional resource directory and assists with managing the waiting list for services.  Ali also develops state and federal reports for the ADRC programs.

Paula Jones

Social Services Case Manager
Paula provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high quality services are provided in a timely and cost effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs.  Paula covers Livingston and Crittenden Counties.

Payton Kidd

Participant Directed Services Coordinator
Payton serves as the coordinator for the Medicaid Home and Community Based Waiver Programs which include the Participant Directed Services / HCB2 Program. He also serves as the coordinator for the Pennyrile Veteran Directed Home and Community Based Services Program (VDHCBS). He screens and processes all referrals as well as provides in-home case management and training to families enrolled in the HCBW and VDHCBS programs. Upon request, he is available to provide community education and training regarding the Participant Directed Services programs.

Rylee Massie

Social Services Case Manager
Rylee provides case management to clients enrolled in the Participant Directed Services Program/Home and Community Based Waiver. She meets with clients in their homes to conduct program visits and training.  She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Harley McCarty

Social Services Case Manager / ADRC Waiver Application Specialist
Harley provides case management services to clients enrolled in the Participant Directed Services/ HCB2, and Traditional HCB2 Programs. Harley meets with clients in their homes to provide monthly case management contacts. She also coordinates training to clients and their caregivers. Harley is the point of contact for Medicaid waiver applications.

Jennifer Medeiros

Social Services Case Manager
Jennifer provides case management to clients enrolled in the Home and Community Based Waiver Program.  She meets with clients in their homes to conduct program visits and training.  She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Heather Meeks

Social Services Case Manager
Heather provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high quality services are provided in a timely and cost effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs.  Heather covers Caldwell and Lyon counties.

Kayla Milauskas

Social Services Case Manager
Kayla provides case management to clients enrolled in the Participant Directed Services Program/Home and Community Based Waiver. She meets with clients in their homes to conduct program visits and training.  She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Amanda Monroe

Social Services Case Manager
Amanda provides case management services to clients enrolled in the Homecare and Older Americans Act programs. She conducts assessments, home visits and conducts resource referrals on behalf of clients. Amanda also serves as the backup for the Aging and Disability Resource Center where she provides intake and prescreening to individuals accessing services.

Marcie O'Neal

Suicide Prevention Task Force Coordinator/Kentucky Suicide Prevention Project
As the Suicide Task Force Coordinator, Marcie plays a critical role in coordinating efforts aimed at prevention and supporting individuals at risk. She is responsible for fostering state and regional partnerships, leading tasking force meetings, providing training, intervention and outreach. She collaborates with grant stakeholders to develop strategic plans and goals for suicide prevention efforts based on community needs and available resources. Marcie assists the AAAIL Director in the collection, analysis of data related to suicide rates, risk factors and prevention efforts.

Amanda Stokes

Assistant AAAIL Director
Amanda assists the AAAIL Director with development of the Area Plan, program budgets, procurement and provider subcontracts. She is responsible for monitoring senior centers, and implementing nutrition guidelines and menu planning.  She provides staffing services for the Aging Advisory Council which includes scheduling of quarterly programs and annual training.  Amanda also serves as case management supervisor and coordinates all in home services.  Amanda is also the network administrator for the data collection system.

Cindy Tabor

Long Term Care Ombudsman
The Ombudsman acts as an advocate for long-term care and group family home residents by investigating and resolving complaints received and assists the Long Term Care Advisory Council.  Cindy also recruits, trains and supervises certified volunteers and friendly visitors, and is the point of contact for the Elder Abuse Council and Forgotten Angels Project.

Stephanie Thompson

Social Services Case Manager / Special Projects Assistant
Stephanie provides case management services to clients enrolled in the Homecare and Older Americans Act programs. She conducts assessments, home visits, and resource referrals for clients. Stephanie also assists with coordinating special projects and activities for the Area Agency on Aging and Independent Living.

Lorrin Washington

Caregiver Programs Support Specialist
Lorrin assists in the development, coordination and implementation of programs for Grandparents Raising Grandchildren and services that support caregivers. This role involves working closely with caregivers, families, healthcare professionals and community organizations to provide resources, support and education to enhance the well being of caregivers.  Lorrin will coordinate and schedule educational workshops, support groups and training sessions. She also provides assessments to determine needs and recommend appropriate resources.

Lydia Watkins

Lydia provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high quality services are provided in a timely and cost effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs.  Lydia covers Christian and Trigg Counties.

Kylie Watson

She meets with clients in their homes to conduct program visits and training.  She provides case management on a monthly basis, coordinates services and provides linkage to community resources.

Miranda White

National Family Caregiver Program Coordinator
Miranda is responsible for the day to day implementation of the National Family Caregiver Program, including home assessments, case management and facilitating local Alzheimer’s support groups.  She also provides assistance to the LTC Ombudsman program.

Kimberly Wiley

Social Services Case Manager
Kim provides case management services to clients enrolled in the Homecare and Older Americans Act programs.  She conducts assessments for elderly and disabled individuals who are seeking in-home support services, ensures access to the continuum of care, ensures appropriate services are provided, ensures the changing needs of the clients and their caregivers are addressed, assures that high-quality services are provided in a timely and cost-effective manner, and refers clients to appropriate formal/informal agencies to help address any unmet needs.  Kim covers Todd and Muhlenberg counties.

Shannon Wynn

Social Services Case Manager/Assessment Specialist/Certified Long Term Care Ombudsman
Shannon conducts comprehensive assessments to identify clients’ needs, strengths, challenges and goals to determine the most suitable supportive services, resources and interventions to effectively meet their needs. Shannon plays a vital role in supporting individuals and families in accessing resources to improve their quality of life.  He also serves as a backup Certified Long Term Care Ombudsman.
